Community Needs Assessment Survey

Sacred Heart Community Service (SHCS) is conducting a survey of public and private service providers in Santa Clara County to enhance our collective work against poverty.  The participation and insight of all providers in the community is extremely valuable. We hope you will complete the survey on behalf of those you serve. 

Who should complete the survey

This survey is intended for public and private organizations that provide any type of services that benefit low-income communities in Santa Clara County. It is best completed by someone in the organization who has a comprehensive understanding of the full range of programs and services the organization provides.

Purpose of the survey

Through the survey, we seek to capture current information on the services and supports organizations provide to address barriers to economic self-sufficiency and conditions related to poverty.

Conditions have changed dramatically in the past 6 months and we want to learn how this has impacted your organization and its customers/clients. The survey includes multiple dimensions:

  • Collaborative practices and partners
  • Service types and trends
  • Resource availability including gaps and opportunities for improving access and availability
  • Customer/client demographics

Purpose of the Community Action Agency

Last summer, SHCS was selected as the Community Action Agency (CAA) for Santa Clara County. The CAA status means that we work with community leaders and organizations to develop and advance a regional campaign that improves the quality of life for our most underserved. As the CAA, we will use information from this survey to advocate on behalf of all of us for resources and systems changes that will benefit low-income community members.

Sharing and using the survey findings

As someone who is deeply involved in this work, you understand how important it is to have current information about how organizations are working individually and collectively. While this survey is part of the State Community Information Profile and Community Needs Assessment reporting requirements, with a strong response from organizations throughout the county, we can also use it to inform ongoing community-wide efforts. For example, we intend to use this information to help update centralized databases so that we are all better able to connect people with needed resources. We will also share with our partners the findings from this survey, along with other research about current conditions and resources in Santa Clara County, so that all of our organizations can use this information to support strategic decision-making and communications.

The more agencies that respond to this survey, the more accurate and relevant the information will be for all of us.
